WHO WE ARE:
The Vista Program is a full service partnership (FSP) program serving up to 180 adults ages 18-59 in the community. The Vista Program offers recovery-oriented intensive outpatient mental health services that provide individuals served with opportunities to utilize their strengths and abilities to gain independence and self–sufficiency in the community.

Referrals are sent directly from Fresno County to the program. Once the individual is referred they are contacted by our Intake specialist to begin the admission process. The intake specialist will meet with the individual and set up an appointment for an initial assessment with the clinician and Psychiatrist.

The target population served includes adults residing in Fresno County, who meet requirements for Serious Mental Illness and meet one of more of the following criteria:
Homelessness
At Risk of Homelessness
Involvement In The Criminal Justice System
Frequent Users of Hospitals And/Or Emergency Room Services.
The Vista Program also provides services for referrals who qualify for Assisted Outpatient Treatment, up to 20 adults.

Vista is funded by the Mental Health Services Act, and serves those suffering from a severe mental illness (SMI) The Program can support up to 180 clients at a time.

RECENT OUTCOMES:
This year, Vista has been able to reduce psychiatric hospitalizations by 70%, incarcerations by 99%, and reduced experience with homelessness by 89%. Vista was able to help 5% of our program participants to engage in an educational setting and employment.
Juan’s Story
Juan entered treatment with Vista due to excessive hospitalizations and suicidal ideation. His family also asked him to move out due to his mental health symptoms, and for a time he was homeless. Through treatment he was able to reduce frequent hospitalization, connect with a case manager, and attend groups in his preferred language. He is now in stable housing, attends the gym, and has not returned to the hospital in the last 3-4 months.
